Run one single-point simulation, repeat the right response once, capture one proctor row, and leave the unit with a trend signal instead of a paperwork burden.

The proctor reads the scene, names the missed step, asks the judgment question, gives the right response, names the threshold, and reruns the moment once in plain language.

Select a sim, start the timer, follow the seven-step proctor script, then generate the CSV row.

- Read the scene aloud.
- State the missed step.
- Ask the judgment question. Take two or three quick answers.
- Read the right response.
- State the threshold to call for help.
- State repair and log: patient check-in, team check-in, one row.
- Rerun the scene once with the right response.
Capture Row
Pick by what the unit is seeing.
The bank is grouped by judgment point: de-escalation, team and coverage, threshold and restraint, and treatment-plan continuity.
The data burden stays one row.
The signal comes from repetition: how often sims are run, how cleanly the loop is followed, and which judgment points keep producing uncertainty.
Sims per week
Count proctor rows by week. A two-week drop tells the unit manager cadence has slipped.
